Care Access is not a covered entity under the Health Insurance Portability and Accountability Act (“HIPAA”). Care Access provides health screening services but does not provide formal diagnosis, treatment, or ongoing medical care and does not replace your primary healthcare provider.

We are committed to protecting the privacy of any personal health information that you provide to us for our screening services. This notice describes how information about you may be used and disclosed and how you can access this information. This applies only to information obtained through our commercial health screening services. Other information we collect, including all information collected through our website, is subject to our Privacy Policy or, if you have signed an Informed Consent Form, the Informed Consent Form you have signed will apply.

We may also contact you if you are potentially eligible for screening for a clinical trial that may be a good fit for you.   • De-identified Information. We may use your health information to create information that does not identify you personally. We may use or share de-identified information with third parties to improve community health, evaluate programs, or support research.
  • Public Health and Safety. We may disclose information to public health authorities when required or permitted by law, for example, to prevent or control disease, report test results of public health importance, or notify individuals at risk of exposure.
  • To Avert a Serious Threat. We may disclose information if necessary to prevent or lessen a serious threat to health or safety.
  • For Other Purposes As Authorized. We will use or disclose your health information for other purposes, as authorized verbally or in writing. For example, if you agree to be evaluated for a clinical trial, we will transfer your information to the clinical trial site with your verbal or written permission.
